กัญชากับโรคที่มีความเสื่อมของระบบประสาทในผู้สูงอายุ

Cannabis in Neurodegenerative Disorders

Abstract

ปัจจุบันมีจำนวนผู้สูงอายุที่เพิ่มขึ้น จึงพบโรคที่เกิดจากความเสื่อมได้มากขึ้น โดยเฉพาะความเสื่อมของระบบประสาท ได้มีความพยายามค้นหาตัวยาหรือสารใหม่ ๆ เพื่อมาใช้ในการบรรเทาหรือรักษาภาวะความเสื่อมต่าง ๆ กัญชาเป็นพืชชนิดหนึ่งที่มีคุณลักษณะที่มีผลต่อการรักษา สารหลักที่สามารถสกัดจากกัญชาประกอบด้วย tetrahydrocannabinol (THC) และ cannabidiol (CBD) โดยออกฤทธิ์ผ่าน cannabinoid receptor 1 (CB1) และ cannabinoid receptor 2 (CB2)ที่มีในมนุษย์ สารทั้งสองหากใช้ในอัตราส่วนที่ไม่เหมาะสมจะมีผลต่ออาการทางจิตประสาท ทางเดินอาหาร และปริชานพิสัย โรคที่เกี่ยวกับความเสื่อมของระบบประสาทในผู้สูงอายุในบทความนี้ ประกอบด้วย ภาวะสมองเสื่อม ปัญหาพฤติกรรม อารมณ์ และจิตใจในผู้ป่วยสมองเสื่อม และ โรคพาร์กินสัน ซึ่งการศึกษาในภาวะดังกล่าวทั้งหมดมีจำนวนประชากรน้อย มีลักษณะคุณภาพงานวิจัยต่ำ และเป็นลักษณะการทดลองแบบเปิด อย่างไรก็ตาม ผลที่ได้จากการศึกษาที่มีพบว่ากัญชาไม่มีผลต่อปริชานพิสัย และไม่ลดปัญหาพฤติกรรม อารมณ์และจิตใจในผู้ป่วยสมองเสื่อม ส่วนผลต่อโรคพาร์กินสันอาจจะช่วยลดผลข้างเคียงที่เกี่ยวกับการเคลื่อนไหวที่ผิดปกติที่เกิดจากยาที่ใช้รักษาแต่ไม่ช่วยลดอาการผิดปกติของเคลื่อนไหวจากตัวโรคเอง ซึ่งก็เป็นการศึกษาที่มีคุณภาพงานวิจัยต่ำ

Currently, increasing aging population leads to face with more neurodegenerative disease. There have been efforts to find new drugs or chemicals to alleviate or treat this problem. Cannabis is one of herbs which can be used in the treatment. There are two major types of cannabinoids including tetrahydrocannabinol (THC) and cannabidiol (CBD) which act by binding with cannabinoid receptors 1 and 2. When the 2 compounds are mixed in inappropriate ratio, they might lead to psychoactive, gastrointestinal and cognitive side effects. In this article, the neurodegenerative conditions reviewed are dementia, Behavioral and Psychological Symptoms of Dementia (BPSD) and Parkinson’s disease. There are a few limitations in the studies in these conditions including low quality of studies, small number of populations and open label studies. However, the available evidence show that cannabis has no effect on cognitive function and BPSD in people with dementia. In Parkinson’s disease, cannabis reduced the drug-induced dyskinesia but not motor symptoms from Parkinson’s disease itself. Nevertheless, the studies were also rated as low quality.
