การธำรงกำลังคนด้านสุขภาพสำหรับการให้บริการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิง

Work Satisfaction and Retention of the Health Workforce for the Long Term Care Services for the Dependent Elderly

Abstract

การศึกษานี้มีวัตุประสงค์เพื่อศึกษาความพึงพอใจต่อการจ้างงานและการคงอยู่ของกำลังคนด้านสุขภาพในการให้บริการผู้สูงอายุที่มีภาวะพึ่งพิงในสถาบันและที่บ้าน กลุ่มตัวอย่างในการวิจัยประกอบด้วยสถานบริการดูแลระยะยาว 66 แห่ง กำลังคนให้บริการในระดับสถาบันซึ่งเป็นกลุ่มวิชาชีพและผู้ช่วยจำนวน 371 คน สำหรับการดูแลที่บ้าน กลุ่มตัวอย่างประกอบด้วยผู้จัดการระบบ 130 คน และนักบริบาล 298 คน เครื่องมือเก็บข้อมูลประกอบด้วยแบบสอบถามที่กรอกด้วยตนเอง โดยเนื้อหาในแบบสอบถามประกอบด้วย ประเภทการจัดบริการ กำลังคนและการจ้างงาน การคงอยู่ ความพึงพอใจในการปฏิบัติงาน ผลการวิจัยพบว่าสถานบริการรองรับผู้สูงอายุมีทั้งภาครัฐและเอกชน ซึ่งผู้ให้บริการเป็นบุคลากรกลุ่มวิชาชีพและผู้ช่วย บุคลากรวิชาชีพ ได้แก่ แพทย์ พยาบาลวิชาชีพ นักกายภาพบำบัด แพทย์แผนไทย นักกิจกรรมบำบัด นักจิตวิทยา บุคลากรระดับผู้ช่วยได้แก่ผู้ช่วยพยาบาล นักบริบาลและพี่เลี้ยง กลุ่มวิชาชีพมีการคงอยู่ในระบบสูงและพึงพอใจต่อการจ้างงานในหลายๆ ประเด็น โดยเฉพาะในด้านการบริหารจัดการ ลักษณะงาน การยอมรับนับถือ ส่วนกำลังคนผู้ช่วยมีการเคลื่อนย้ายออกจากองค์กรสูง (31-48%) อย่างไรก็ตามนักบริบาลให้บริการเยี่ยมบ้านมีอัตราการลาออกต่ำกว่า (12%) ในด้านความพึงพอใจในการปฏิบัติงาน นักบริบาลพึงใจค่อนข้างสูงเกี่ยวกับ ความสัมพันธ์ที่ดีกับเพื่อนร่วมงาน รู้สึกมีความสุขที่ได้ดูแลผู้ป่วย การได้รับการยอมรับจากผู้ป่วยและญาติ แต่มีความพึงพอใจต่ำในด้านค่าตอบแทน การอบรมเพิ่มพูนทักษะ ภาระงานมาก ดังนั้นมีความจำเป็นที่ต้องธำรงกำลังคนคนในระบบโดยลดการสูญเสียและการสร้างแรงจูงใจที่เหมาะสม โดยเฉพาะนักบริบาล การสร้างแรงจูงใจนั้นควรผสมผสานระหว่างค่าตอบแทนที่เหมาะสม สิทธิประโยชน์อื่น ๆ และ โอกาสความก้าวหน้าในอาชีพด้วย

This study aimed to assess the retention and work satisfaction of health workforce working in long term care (LTC) services at institutional and home health care. Sample size comprise 66 LTC institutions, and 371 health workforce, both health professional and care assistants. In additional, 130 care managers and 298 care givers responsible for home health care were included as sample size. The self-administered questionnaires comprised questions in relation to service provision, health workforce and employment, retention, work satisfaction. The results revealed that health workforce providing LTC at institutions composed of registered nurses, social workers, doctors, physiotherapists, Thai traditional practitioners, occupational therapists, and psychologists. Care assistants comprised nurse assistants, care givers and general assistants. For health professional, retention rate was high. Work satisfaction was high particularly in relation to health workforce management, work itself and social acceptance. Turnover of care assistants working at LTC institutions was high (31-48%), however, that of care givers providing care at home was lower (12%). For care assistants, high work satisfaction was found in the areas of good relationship with co-workers, good attitude towards patients, acceptance by patients and relatives. However, low work satisfaction was in relation to low income, continuing training and high workload. Thus, it is crucial to retain health workforce, particularly care assistants by reducing turnover rate. Combination of financial and non-financial incentives were suggested in order to retain health workforce.
