The Development of Clinical Nursing Practice Guideline for Non- Pharmacological Therapy Pain Management with Pharmacological Pain Management in Community Care for Older Adults with Cancer, Rattanaburi District, Surin Province.

Abstract

Situation of the older adults in Thailand has increased. 41.38 percent of the cases with diagnosis of cancer constitute older adults. Rattanaburi District, Surin Province reported in the 2017 – 2019 were older adults with cancer of 65.63, 61.52 and 59.96 percent, respectively. The illness that most threatens the elderly with cancer is pain. Polypharmacy and age-related changes pharmacokinetics and pharmacodynamics are need to be caution. Pharmacological with Non- Pharmacological method are most effective. This study aimed to develop nursing practice guidelines (CNPG) for non- pharmacological pain management with pharmacological pain management in community care for older adults with cancer. This study was developed using the Soukup model (2000) as a conceptual framework. The results of the evaluation, quality analysis and synthesis create guidelines for nursing as follows: 1) Assessment, included vision, hearing, cognitive, depression, pain; 2) pain management according with physician’s treatment and non-pharmacological pain management chosen by older adults as follows 1. Massage/ Sham Massage 2. Foot Reflexology/ Sham Foot Reflexology 3. Massage with aromatherapy 4. Music Therapy 5. Art Therapy 6. Meditation. 3) Continuing of pain assessment. 5 experts expressed their agreement with the CNPG for 88.04 percent. The 80 percent of older patients had reduced pain average level and elderly and caregivers were 90 percent overall satisfied. In the process was found 80.95 percent of nurse were able to comply this CNPG and 85 percent of nurse satisfaction in Pilot studies. It is recommended that this CNPG be used for the management of pain in older adults with cancer in the community.
