Abstract

ในสถานการณ์การระบาดรุนแรงของโรคติดเชื้อไวรัสโคโรนา 2019 (โควิด-19) ทำให้ระบบสาธารณสุข ของประเทศไม่สามารถรองรับจำนวนผู้ป่วยที่เพิ่มขึ้นอย่างมากและรวดเร็ว ทำให้ผู้ป่วยสูงอายุจำนวนมากไม่สามารถเข้าถึงการรักษาในโรงพยาบาลได้ จำเป็นต้องรับการดูแลแบบแยกกักตัวที่บ้านของตนเอง และอีกจำนวนมากที่จำเป็นต้องได้รับการดูแลแบบกักตัวในชุมชน โดยมีบุคลากรทางสุขภาพที่ให้คำแนะนำปรึกษาทางไกล ปัจจุบันพบว่าบุคลากรทางสุขภาพจำนวนหนึ่งอาจไม่คุ้นเคยกับลักษณะที่จำเพาะของผู้ป่วยสมาคมพฤฒาวิทยาและเวชศาสตร์ผู้สูงอายุไทยจึงร่วมมือกับราชวิทยาลัยเวชศาสตร์ครอบครัวแห่งประเทศไทย และสถาบันเวชศาสตร์สมเด็จพระสังฆราชญาณสังวรเพื่อผู้สูงอายุ กรมการแพทย์ จึงได้จัดทำแนวปฏิบัตินี้เพื่อเสริมกับแนวปฏิบัติที่มีอยู่แล้ว ครอบคลุมตั้งแต่การสังเกตและติดตามอาการระหว่างแยกตัว อาการและสัญญาณชีพที่บ่งถึงลักษณะทางคลินิกที่เลวลง การดูแลทั่วไปและการใช้ยา แนวทางการวินิจฉัยและจัดการภาวะสับสนเฉียบพลัน แนวทางการประเมินและแก้ไขปัญหาด้านโภชนาการ ตลอดจนผลกระทบหลังจากที่ผู้ป่วยหายจากการติดเชื้อ แต่เกิดภาวะถดถอยด้านความสามารถในการดำเนินกิจวัตรประจำวัน จำเป็นต้องได้รับการฟื้นฟูบำบัด

The coronavirus 2019 (Covid-19) pandemic results in disruption of the national health system to serve the enormously and rapidly rising of afflicted patients. This bars older people from receiving treatment in hospitals. Therefore, they need to receive care under home isolation and many of them are under community isolation with telemedicine consultation from health personnel. At the moment, some of the health personnel are not familiar with the unique change in older people. The Thai Society of Gerontology and Geriatric Medicine in-cooperate with the Royal College of Family Physicians of Thailand and the Institute of Geriatric Medicine, Department of Medical Service to produce the guidance on top of current existing guidance. It covers the observation and monitors during isolation, symptoms and vital signs indicating worsening clinical progression, general care, and medication use, diagnosis and management of delirium, assessment and management of the nutritional problem and consequence of post-covid stage with functional decline requiring rehabilitation.
