ความร่วมมือในการใช้ยาของผู้ป่วยสูงอายุ ที่ได้รับการดูแลโดยทีมสหสาขาวิชาชีพในคลินิกสูงวัย

Effect of Multidisciplinary Team on Medication Compliance Among Older Patients in Geriatric Clinic

Abstract

ที่มาและวัตถุประสงค์ : คลินิกสูงวัยโรงพยาบาลลำปาง จัดตั้งขึ้นสำหรับดูแลผู้ที่มีอายุมากกว่า 60 ปีขึ้นไป มีโรคร่วมหลายโรค เป็นคลินิกที่ดูแลผู้ป่วยด้วยทีมสหสาขาวิชาชีพ ประกอบด้วย แพทย์ เภสัชกร พยาบาล นักกิจกรรมบำบัด นักโภชนาการ และนักสังคมสังเคราะห์ โดยดูแลสุขภาพผู้ป่วยแบบองค์รวม เพื่อให้ผู้ป่วยมีความสมบูรณ์ทั้งทางร่างกาย จิตใจ สังคม และจิตวิญญาณ ส่งผลให้ผู้ป่วยมีการดำเนินชีวิตที่ยืนยาวร่วมกับการมีคุณภาพชีวิตที่ดี ซึ่งผู้สูงอายุที่มีโรคเรื้อรังจำเป็นต้องใช้ยาในการรักษา ดังนั้นความร่วมมือในการใช้ยาจึงมีส่วนสำคัญในการรักษาผู้ป่วยให้ได้ตามเป้าหมาย การศึกษานี้มีวัตถุประสงค์ เพื่อศึกษาความร่วมมือในการใช้ยาและปัจจัยที่สัมพันธ์กับความร่วมมือในการใช้ยาในผู้ป่วยคลินิกสูงวัย โรงพยาบาลลำปาง เมื่อได้รับการดูแลจากทีมสหสาขาวิชาชีพ วิธีการวิจัย : เป็นการศึกษาแบบภาคตัดขวาง ในผู้สูงอายุที่เข้ารับการรักษาในคลินิกสูงวัย โรงพยาบาลลำปาง ตั้งแต่เดือนมิถุนายน พ.ศ. 2561 จนถึงธันวาคม พ.ศ. 2563 เก็บข้อมูลผู้ที่เข้ารับบริการเป็นรายครั้งในคลินิกสูงวัย โดยใช้แบบเก็บข้อมูลประกอบด้วยข้อมูลพื้นฐานของผู้ป่วย ความร่วมมือในการใช้ยา ซึ่งหมายความถึงใช้ยาตามแพทย์สั่ง ไม่หยุดใช้ยาเอง หรือลืมใช้ยาน้อยกว่าหรือเท่ากับ 2 ครั้ง/สัปดาห์ วิเคราะห์ข้อมูลทั่วไปของผู้ป่วยด้วยสถิติเชิงพรรณนา และวิเคราะห์ปัจจัยที่สัมพันธ์กับความร่วมมือในการใช้ยาโดยสถิติ Chi-square test และ Multivariable logistic regression ผลการศึกษา : ผู้สูงอายุทั้งหมด 433 คน เข้ารับการบริการคลินิกสูงวัย 1,364 ครั้ง ค่ามัธยฐานการมาติดตามการรักษา เท่ากับ 2 (IQR: 1,4) ครั้ง มีอายุเฉลี่ย 79.3 + 8.3 ปี มีผู้ดูแลการใช้ยา 182 คน (ร้อยละ 42) มีโรคประจำตัวเฉลี่ยคนละ 3.0 ± 1.2 โรค พบให้ความร่วมมือในการใช้ยาจำนวน 1,025 ครั้ง (ร้อยละ 75.1) โดยผู้สูงอายุที่มีผู้ดูแลเรื่องการใช้ยามีความร่วมมือในการใช้ยา 1.35 เท่า (OR =1.35, 95%CI: 1.04-1.75) เมื่อเทียบกับผู้สูงอายุที่ไม่มีผู้ดูแลเรื่องการใช้ยาผู้สูงอายุที่เป็นโรคหัวใจมีความร่วมมือในการใช้ยา 1.66 เท่า (OR =1.66, 95%CI: 1.13-2.44) เทียบกับผู้สูงอายุที่ไม่เป็นโรคหัวใจ แต่ผู้สูงอายุที่เป็นโรคเบาหวานมีความร่วมมือในการใช้ยา 0.63 เท่า (OR =0.63, 95%CI: 0.48-0.83) เมื่อเทียบกับผู้สูงอายุที่ไม่เป็นโรคเบาหวาน และผู้สูงอายุที่เป็นโรคหืดและปอดอุดกั้นเรื้อรังมีความร่วมมือในการใช้ยา 0.34 เท่า (OR =0.34, 95%CI: 0.24-0.49) เมื่อเทียบกับผู้สูงอายุที่ไม่เป็นโรคหืดและปอดอุดกั้นเรื้อรัง สรุปผลการศึกษา : คลินิกสูงวัย โรงพยาบาลลำปางด้วยความร่วมมือของสหสาขาวิชาชีพสามารถสนับสนุนความร่วมมือในการใช้ยาของผู้สูงอายุได้เป็นอย่างดี และควรมีมาตรการในการดูแลผู้สูงอายุโดยเฉพาะผู้สูงอายุที่ไม่มีผู้ดูแลการใช้ยาผู้ป่วยโรคเบาหวาน โรคหืดและปอดอุดกั้นเรื้อรัง เพื่อเพิ่มความร่วมมือในการใช้ยาและส่งผลให้ผู้สูงอายุสามารถรักษาโรคได้ตามเป้าหมาย

Background and Objectives: Geriatric clinic in Lampang hospital is established to look after older patients with multiple comorbidities. This clinic takes care older patients with a multidisciplinary team consisting of doctor, pharmacist, nurse, occupational therapist, nutritionist and social worker to provide holistic care to provide patients with complete physical, mental, social, and spiritual integrity resulting in patients living a long life with a good quality of life older people take a lot of medicine, so compliance is very important to achieve goal of therapy. This study aimed to evaluate medication compliance and factors associated medication compliance among older at Geriatric Clinic in Lampang hospital. Method: This study was a cross-sectional study of older people at Geriatric Clinic in Lampang hospital between June 2018 and December 2020. Data were collected by medical record form, including baseline characteristics, and medication compliance. Baseline characteristics were analyzed by descriptive statistics and the factors associated with medication compliance were evaluated using Chi-square test and multivariable logistic regression. Results: A total of 433 older people attended the older clinic 1,364 visiting times in this study. The median follow-up time was 2 (IQR: 1,4) times. The mean age of patients was 79.3 ± 8.3 years. The older who living with care giver was 182 people (42%). The mean of comorbidities was 3.0 ± 1.2 diseases. Good medication compliance was 1,025 visiting times (75.1%). Older with care giver were more 1.35 times (ORadj=1.35, 95%CI: 1.04-1.75), likely to have good medication compliance, compared to elderly without care giver. Heart disease patients were more 1.66 times (ORadj=1.66, 95%CI: 1.13-2.44) likely to have good medication compliance, compared to those who were without heart disease. Diabetic patients were less medication compliance than 0.63 times (ORadj=0.63, 95%CI: 0.48-0.83), compared to older without diabetes mellitus. Older with asthma and chronic obstructive pulmonary disease were less medication compliance than 0.34 times (ORadj=0.34, 95%CI: 0.24-0.49), compared to elder without asthma and chronic obstructive pulmonary disease. Conclusions: The geriatric clinic in Lampang hospital, with the cooperation of the multidisciplinary team, can support medication compliance for the e older. And there should be a procedure to take care of the older, especially those who do not have a caregiver, patients with diabetes mellitus and asthma or chronic obstructive pulmonary disease, to increase medication compliance and result in the older being able to achieve the goal of therapy.
